This is a plugin that is linked to a project using a Data Entry Trigger. It allows users to configure multiple 'conditions' that can trigger an email alerts, such as a suicidal response to a survey.

Currently there is only one 'alert email' that can be configured, but I hope to add the ability to configure separate alerts for each test condition in the future.
